A Donegal TD says many hospital patients who are medically discharged from hospitals cannot go home because of lack of supports.
Fianna Fail Deputy Pat The Cope Gallagher says the problem is becoming worse because of ongoing delays in the approval of home care packages.
He says there is no joined up thinking between acute hospitals, community hospitals and those involved in the provision of home care.
